Request for and Consent to Release of Information from Individual's Records
Authorizes the VA to release your VA records to someone you choose.
Opens the official VA.gov page, always the authority on the current version and instructions.
Where 21-4142 lets the VA collect records from others, this form works the other direction: it authorizes the VA to release information from your records to a third party you name, a lawyer, a lender, or anyone else you designate.
